Faculty Media Request Form

The WSU Libraries Media Collection is housed in the David Adamany Undergraduate Library (UGL). A primary purpose of the collection is to help support classroom instruction. Any WSU faculty requests to purchase Videos, DVDs, CDs and other media required for limited classroom instruction should go through the Media Librarian Diane Sybeldon. The purpose of this form is to ensure the availability of a particular title(s) at the time needed for your classroom session. For instance, you may request a title in September to be retrived and held for you by a specified date in October. If you have questions regarding this service please contact Marilyn Weiss at the Undergraduate Library’s Check Out desk.

NOTE: This is not a Course Reserve form. Please do not use this form to request media for Course Reserve. Use the Course Reserve order form at http://www.lib.wayne.edu/forms/reserves_request.php

Procedure:

  • Please check availability of title in the library catalog before placing request.
  • Complete the form below.
  • Your request will be processed as soon as received by library staff.
  • The title requested will be set aside 24 hours prior to the pickup date.
  • All requested media MUST be picked up and returned to the Check Out Desk of the UGL. Items will not be shipped via mail to offices and/or residence either on or off campus.

Please help us by doing the following:

  • Fill out the form below as completely as possible.
  • Request only one title per form. For multiple titles submit the form and then return to the form page.
  • Plan ahead. Try to place your request at least one month before the scheduled classroom showing.
  • Have backup plans. Provide us with two to three possible viewing dates. With this scheduling flexibility, when a title is not available for the first date, you may still have the opportunity to show it during the term.

The library cannot guarantee that we will purchase a title for a classroom showing that is not found in our library catalog.

Possession of a film or video does not confer the right to show the work. The copyright owner specifies, at the time of purchase or rental, the circumstances in which a film or video may be "performed". For example, videocassettes from a video rental outlet usually bear a label that specifies "Home Use Only". However, whatever their labeling or licensing, use of this media is permitted in an educational institution so long as certain conditions are met.

Section 110 (1) of the Copyright Act of 1976 specifies that the following is permitted:

Performance or display of a work by instructors or pupils in the course of face-to-face teaching activities of a nonprofit educational institution, in a classroom or similar place devoted to instruction, unless, in the case of a motion picture or other audiovisual work, the performance, or the display of individual images is given by means of a copy that was not lawfully made...and that the person responsible for the performance knew or had reason to believe, was not lawfully made.

Additional text of the Copyright Act and portions of the House Report (94-1476) combine to provide the following, more detailed list of conditions [from Virginia M. Helms, supra]:

  1. They must be shown as part of the instructional program.
  2. They must be shown by students, instructors, or guest lecturers.
  3. They must be shown either in a classroom or other school location devoted to instruction such as a studio, workshop, library, gymnasium, or auditorium if it is used for instruction.
  4. They must be shown either in a face-to-face setting or where students and teacher(s) are in the same building or general area.
  5. They must be shown only to students and educators.
  6. They must be shown using a legitimate (that is, not illegally reproduced) copy with the copyright notice included.

Further, the relationship between the film or video and the course must be explicit. Films or videos, even in a "face-to-face" classroom setting, may not be used for entertainment or recreation, whatever the work's intellectual content.
